Over 100 million Americans suffer from chronic pain, which causes more disability than any other medical condition in the United States at a cost of $560–$635 billion per year. Opioid analgesics are frequently used to treat CP. However, long term use of opioids can cause brain changes such as opioid-induced hyperalgesia that, over time, increase pain sensation. Also, opioids fail to treat complex psychological factors that worsen pain-related disability, including beliefs about and emotional responses to pain. Cognitive behavioral therapy can (...) be efficacious for CP. However, CBT generally does not focus on important factors needed for long-term functional improvement, including attainment of personal goals and the psychological flexibility to choose responses to pain. Acceptance and Commitment Therapy has been recognized as an effective, non-pharmacologic treatment for a variety of CP conditions. However, little is known about the neurologic mechanisms underlying ACT. We conducted an ACT intervention in women with chronic musculoskeletal pain. Functional magnetic resonance imaging data were collected pre- and post-ACT, and changes in functional connectivity were measured using Network-Based Statistics. Behavioral outcomes were measured using validated assessments such as the Acceptance and Action Questionnaire, the Chronic Pain Acceptance Questionnaire, the Center for Epidemiologic Studies Depression Scale, and the NIH Toolbox Neuro-QoLTM scales. Results suggest that, following the 4-week ACT intervention, participants exhibited reductions in brain activation within and between key networks including self-reflection, emotion, and cognitive control. These changes in connectivity strength were correlated with changes in behavioral outcomes including decreased depression and pain interference, and increased participation in social roles. This study is one of the first to demonstrate that improved function across the DMN, SN, and FPN may drive the positive outcomes associated with ACT. This study contributes to the emerging evidence supporting the use of neurophysiological indices to characterize treatment effects of alternative and complementary mind-body therapies. (shrink)

At the time of this writing, the late Myles Brand was the President of Indiana University and a member of the IU Department of Philosophy.

Throughout the biological and biomedical sciences there is a growing need for, prescriptive ‘minimum information’ (MI) checklists specifying the key information to include when reporting experimental results are beginning to find favor with experimentalists, analysts, publishers and funders alike. Such checklists aim to ensure that methods, data, analyses and results are described to a level sufficient to support the unambiguous interpretation, sophisticated search, reanalysis and experimental corroboration and reuse of data sets, facilitating the extraction of maximum value from data sets (...) them. However, such ‘minimum information’ MI checklists are usually developed independently by groups working within representatives of particular biologically- or technologically-delineated domains. Consequently, an overview of the full range of checklists can be difficult to establish without intensive searching, and even tracking thetheir individual evolution of single checklists may be a non-trivial exercise. Checklists are also inevitably partially redundant when measured one against another, and where they overlap is far from straightforward. Furthermore, conflicts in scope and arbitrary decisions on wording and sub-structuring make integration difficult. This presents inhibit their use in combination. Overall, these issues present significant difficulties for the users of checklists, especially those in areas such as systems biology, who routinely combine information from multiple biological domains and technology platforms. To address all of the above, we present MIBBI (Minimum Information for Biological and Biomedical Investigations); a web-based communal resource for such checklists, designed to act as a ‘one-stop shop’ for those exploring the range of extant checklist projects, and to foster collaborative, integrative development and ultimately promote gradual integration of checklists. BackgroundThe boundaries between health-related research and practice have become blurred as initiatives traditionally considered to be practice increasingly use the same methodology as research. Further, the application of different ethical requirements based on this distinction raises concerns because many initiatives commonly labelled as “non-research” are associated with risks to patients, participants, and other stakeholders, yet may not be subject to any ethical oversight. Accordingly, we sought to develop a tool to facilitate the systematic identification of risks to human participants and (...) determination of risk level across a broad range of projects and health-related contexts. This paper describes the development of the Public Health Ontario Risk Screening Tool.MethodDevelopment of the PHO Risk Screening Tool included: preparation of a draft risk tool ; expert appraisal; internal stakeholder validation; external validation; pilot testing and evalution of the draft tool; and revision after 1 year of testing.ResultsA risk screening tool was generated consisting of 20 items organized into five risk domains: Sensitivity; Participant Selection, Recruitment and Consent; Data/Sample Collection; Identifiability and Privacy Risk; and Commercial Interests. The PHO Risk Screening Tool is an electronic tool, designed to identify potential project-associated risks to participants and communities and to determine what level of ethics review is required, if any. The tool features an easy to use checklist format that generates a risk score associated with a suggested level of ethics review once all items have been completed. The final score is based on a threshold approach to ensure that the final score represents the highest level of risk identified in any of the domains of the tool.ConclusionsThe PHO Risk Screening Tool offers a practical solution to the problem of how to maintain accountability and appropriate risk oversight that transcends the boundaries of research and practice. We hope that the PHO Risk Screening Tool will prove useful in minimizing the problems of over and under protection across a wide range of disciplines and jurisdictions. (shrink)

Consent remains a crucial, yet challenging, cornerstone of clinical practice. The ethical, legal and professional understandings of this construct have evolved away from a doctor-centred act to a patient-centred process that encompasses the patient’s values, beliefs and goals. This alignment of consent with the philosophy of shared decision-making was affirmed in a recent high-profile Supreme Court ruling in England. The communication of information is central to this model of health care delivery but it can be difficult for doctors to gauge (...) the information needs of the individual patient. The aim of this paper is to describe ‘core information sets’ which are defined as a minimum set of consensus-derived information about a given procedure to be discussed with all patients. Importantly, they are intended to catalyse discussion of subjective importance to individuals. The model described in this paper applies health services research and Delphi consensus-building methods to an idea orginally proposed 30 years ago. The hypothesis is that, first, large amounts of potentially-important information are distilled down to discrete information domains. These are then, secondly, rated by key stakeholders in multiple iterations, so that core information of agreed importance can be defined. We argue that this scientific approach is key to identifying information important to all stakeholders, which may otherwise be communicated poorly or omitted from discussions entirely. Our methods apply systematic review, qualitative, survey and consensus-building techniques to define this ‘core information’. We propose that such information addresses the ‘reasonable patient’ standard for information disclosure but, more importantly, can serve as a spring board for high-value discussion of importance to the individual patient. The application of established research methods can define information of core importance to informed consent. Further work will establish how best to incorporate this model in routine practice. Comparative Political Philosophy: Studies Under the Upas Tree examines four major traditions of political philosophy and discusses similarities in their key ideas and assumptions. Understanding spoken words involves a rapid mapping from speech to conceptual representations. One distributed feature-based conceptual account assumes that the statistical characteristics of concepts’ features—the number of concepts they occur in and likelihood of co-occurrence —determine conceptual activation. To test these claims, we investigated the role of distinctiveness/sharedness and correlational strength in speech-to-meaning mapping, using a lexical decision task and computational simulations. Responses were faster for concepts with higher sharedness, suggesting that shared features are facilitatory in tasks like lexical decision (...) that require access to them. Correlational strength facilitated responses for slower participants, suggesting a time-sensitive co-occurrence-driven settling mechanism. The computational simulation showed similar effects, with early effects of shared features and later effects of correlational strength. These results support a general-to-specific account of conceptual processing, whereby early activation of shared features is followed by the gradual emergence of a specific target representation. We describe on-going work on IAO-Intel, an information artifact ontology developed as part of a suite of ontologies designed to support the needs of the US Army intelligence community within the framework of the Distributed Common Ground System (DCGS-A). IAO-Intel provides a controlled, structured vocabulary for the consistent formulation of metadata about documents, images, emails and other carriers of information. It will provide a resource for uniform explication of the terms used in multiple existing military dictionaries, thesauri and metadata registries, (...) thereby enhancing the degree to which the content formulated with their aid will be available to computational reasoning. Complementing recent studies by Keown, Whitehill, and Hallisey that associate Buddhist ethics with the virtue tradition, the author proposes that Buddhist virtue requires both overcoming attachment to self and compassionate regard for others. Within a broader framework of comparative religious ethics, such a claim is not extraordinary; overcoming prudentialist self-interest, cultivating sympathy, and acting on others' behalf are ethical values highly praised by most religious traditions, including Buddhism. Nevertheless, this proposal runs counter to those who hold Theravāda Buddhism to be (...) dominated by the world-renouncing values of the monk antithetical to a social ethic. It also differs from the view that monastic and lay ethics, while not in opposition, are merely complementary. The author contends that the moral value of voluntary poverty, ordinarily labeled a monastic virtue, can be seen as the foundation for other-regarding virtues such as compassion and benevolence. Scholars studying the ethical, legal, and social issues associated with emerging technologies maintain the importance of considering these issues throughout the research and development cycle, even during the earliest stages of basic research. Embedding these considerations within the scientific process requires communication between ELSI scholars and the community of physical scientists who are conducting that basic research. We posit that this communication can be effective on a broad scale only if it links societal issues directly to characteristics of the emerging (...) technology that are relevant to the physical and natural scientists involved in research and development. In this article, we examine nano-ELSI literature from 2003 to 2010 to discern the degree to which it makes these types of explicit connections. We find that, while the literature identifies a wide range of issues of societal concern, it generally does so in a non-specific manner. It neither links societal issues to particular forms or characteristics of widely divergent nanotechnologies nor to any of the many potential uses to which those nanotechnologies may be put. We believe that these kinds of specificity are essential to those engaged in nano-scale research. We also compare the literature-based findings to observations from interviews we conducted with nanoscientists and conclude that ELSI scholars should add technical- and application-related forms of specificity to their work and their writings to enhance effectiveness and impact in communicating with one important target audience—members of the nanoscale science community. This study of the ethics of Bhikkhu Buddhadāsa, Thailand's foremost interpreter of Theravāda Buddhism, exemplifies the position that (1) religious ethics is to be studied as an aspect of an organically integrated religious system or tradition, and that (2) the field of religious ethics should be conceived primarily as a subset of the field of religious studies or the history of religions, broadly conceived, rather than a subset of such disciplines as philosophy and/or sociology. Descriptively, the article first sets out (...) the broad parameters of Buddhadāsa's worldview; second, analyses three ethical dimensions of this worldview; third, correlates the particular ethical system of Buddhadāsa with a more generalized schematization of Theravāda ethics; and, in conclusion, briefly contrasts this (holistic) approach with those of Max Weber, Winston King, David Little and SumnerTwiss. Diagnosed with cancer over a year ago, he continued many of his usual activities--reading widely, maintaining a voluminous correspondence, visiting with friends, and walking daily. Winston was one of those remarkable scholar-teachers of an older generation who (...) never ceased to develop new intellectual and research interests. With degrees from Asbury College (A.B., 1929), Andover Newton Theological Seminary (1936), and Harvard (S.T.M., 1938; Ph.D., 1940), his career included pastorates in New England (1930-1943, 1945-1949), Dean of the Chapel and professor of Grinnell College (1949-1963), professor of the history of religions atVanderbilt University (1964-1973), and after his retirement fromVanderbilt, an appointment as professor of philosophy at Colorado State University.Winston's two-year appointment with the Ford Foundation as the advisor to the International Institute for Buddhistic Studies in Rangoon (Yangon), Burma (Myanmar), from 1958 to 1960 proved to be a major turning point in his life and established his reputation as a significant interpreter of Theravada Buddhism in Southeast [End Page vi] Asia. SwearerI intend no disrespect to either the Buddha or the Christ by my rewrite of Anna Bartlett Warner’s 1859 Sunday school song, “Jesus Loves Me.” That one might construct the Buddha in the image of a loving Jesus may be more startling or offensive to Buddhists (and also to Christians) than the modern, apologetic view of (...) the Buddha as a rational renouncer. Versions of both can be textually justified; however, each is a reification that reflects the bias of the interpreter. Both ignore the complexity of the figure of the Buddha within the varied and diverse traditions of Buddhism. My starting point in the following essay acknowledges this complexity as well as the inevitable limitations of my personal interpretation of the Buddha, necessarily conditioned by my experience. I propose to structure the essay around two polarities—universal/particular and wisdom/compassion—looking first at the Buddha and then briefly reflecting dialogically at the end of each section on the figure of the Christ.Buddha: Universal and ParticularIn my personal experience in Buddhist Asia, especially Thailand, and my studies of Buddhist traditions, in particular the Theravada, I have been impressed by the creative tension between the universal and particular dimensions of the figure of the [End Page 113] Buddha. Even the modern Western view of the Buddha evidences this tension. Typically the word Buddha evokes the story of Prince Siddhattha’s renunciation of his royal status, subsequent quest for enlightenment, and eventual realization of nibbana. 1 In the hands of a comparative mythologist such as Joseph Campbell, the detailed embellishments of the Buddha’s sacred biography are absorbed into the tripartite, monomyth structure of the hero’s narrative—separation, attainment, return. For Campbell, the story of the historical Buddha, Siddhattha Gotama, represents a rite of passage, a deep psychological truth of self-discovery symbolized by other heroes in world mythology. Despite the antistructuralist critique by historical and textual scholars, as well as the feminist criticism that the heroic journey does not reflect women’s experience, in one form or another the Buddha reified as an exemplar of the universal paradigm of the individual’s journey of self-realization continues to capture the Western imagination.A somewhat more sophisticated version of the universal hero motif is the Buddha as ‘rational renouncer,’ an interpretation favored by both Buddhists and scholars of Buddhism who portray the Buddha as an empiricist and pragmatist somewhat on the order of William James. In this view, Prince Siddhattha’s quest was a rational response to the experience of suffering, and the truth he perceived on the night of his enlightenment was nothing more nor less than a direct perception of the universal law of cause and effect obscured to ordinary awareness. De-emphasizing both the ascetical and devotional aspects of the Buddha’s sacred biography, the rational renouncer school of thought places its emphasis on epistemological transformation characterized in Buddhist texts as “seeing things as they really are.”The Buddha as universal hero and as rational renouncer can be critiqued as modernized, rationalized, westernized reifications of the Buddha, an example of the ‘orientialist’ distortion that characterized the colonial project. Charles Hallisey has pointed out that such reconstructions of the Buddha were not merely the products of Western scholars but owed much to Asian Buddhists as well. 2 But are we to assume that throughout the history of Buddhism, similar hermeneutical moves and debates over the nature of the Buddha did not exist? I find a tension in the Buddhist tradition between two contrasting images of the Buddha: as an exemplar of a universal truth, and as a being defined by the contingencies of human particularities.Theravada Buddhists find strong support in the sutta for a humanistic interpretation of the Buddha. In particular, they argue that the Buddha of the early Buddhist canonical texts specifically denied that he was a god, and that he was a teacher of the truth he perceived at his enlightenment. The lecture wasn't autobiographical—David was much too unassuming to have thought of himself in these terms—but those of us who loved David and who had the privilege of working with him over many years have no (...) such qualms. David's dedication, sincerity of spirit, generosity, and compassion were truly bodhisattva-like.In his lecture David contrasts the superficial, electronic Internet connectedness of our lives today with the common ground discovered at Awakening based on what David called "heart connection," an insight into the shared nature of all sentient beings that leads to empathy, compassion, and kindness. In contrast to enlightened heart connectedness, Internet interaction lacks the capacity of an intimacy with the varied phenomena of life at each moment. "The Internet," he observes, "allows us to ease our solitude by getting access to things that are compatible with our own interests. However, Buddhism considers that the achievement of enlightenment involves being able to be compatible and have empathy with things that may seem foreign or offensive or a threat to us by seeing them as connected and similar to ourselves.
